To whom it may concern,
This denial is inconsistent with your own Medical Policy Bulletin PT-014 (effective 03/01/2025), which states that therapeutic exercise is covered when “documentation demonstrates objective functional deficits and a skilled treatment plan with measurable goals” (§2.1). The submitted documentation meets each element of §2.1, as itemized below.
Per PT-014 §4.3, this determination should be reversed on first-level review without requiring peer-to-peer.
